Who is Eligible?

Children newborn through 18 years of age.

Children in qualifying middle- or low-income families. For example, a family of four that earns about $58,000 or less a year would be eligible.

Children not covered by insurance through an employer.

Immigration status does not matter.

For the children who meet the eligibility criterion above but who do not qualify for Medi-Cal or Healthy Families and who don’t live in an area covered by Kaiser Child Health Plan.

What is the Cost?

Insurance may be free or at a very low cost.

We will help you apply for the best
insurance program.

Healthy Families Cost

Premiums
Healthy Families applicants pay a monthly premium . The premium is between $4 and $15 for each child — up to a maximum amount of $45 a month for all children in the family.

Co-payments
Healthy Family members also pay a co-payment (usually $5) for most services. Co-payments are payments for visits to the doctor, for prescription drugs, for emergency services or other services. Some services are free. Free services include immunizations and other preventive care.
 
The most a family has to pay in co-payments is $250 per year.

For children of American Indian descent or Alaska Natives, premiums and co-payments may be waived (free).

Kaiser Child Health Plan Cost

Monthly Premiums
Monthly premiums range from $8 to $15 per child, up to three children, depending on income levels. Additional children are covered at no extra premium.
 
Co-Payments
The co-payment limit for the year is $250 for one child, $500 for two children or more.
 
Healthy Kids Cost

Monthly Premiums
Depending on how much a family earns in a year, premiums range from $4 to $12 per child per month, up to three children.


Co-Payments

When you visit a doctor, there may be a small co-payment at the time of the visit.  Well-child visits and shots (immunizations) are free.

Sick child, dental, eye visits, emergency room visits and prescriptions are $5.

How to Enroll?

Certified Application Assistants are available in several Yolo County communities. You will need to bring the following documentation:

  • Proof of income
  • Proof of age of child
  • Proof that you live in Yolo County.

A Certified Application Assistant will answer your questions and help you make the right choice for insuring your child. Every call, every conversation is kept confidential, so your privacy is protected. Our goal is to build trust with you and your family and to be there to help you every step of the way. How to Keep Your Coverage

For Healthy Kids, you will need to re-apply once a year. A re-enrollment packet will be sent to you about 2 1⁄2 months before your year is up.


¿ Quien es Elegible?

¿Como Inscribirse?

Como Mantener su Cobertura

Para Healthy Kids, usted necesitara solicitar una vez al año todos los años. Un paquete de matricula será enviado a usted en aproximadamente dos meses y medio antes de que el año se ve.
